Chronical fermentative inflammation and recurrent neck bladder stricture due to complications of pancreas transplan- tation

A clinical observation of the development of complications in the form of recurrent sclerosis (stricture) of the bladder neck in a patient following pancreas transplantation for type 1 diabetes mellitus with the imposition of duodenocystoanastomosis is presented.Severe enzymuria led to the development of bladder neck sclerosis with clinical symptoms of bladder outlet obstruction, accompanied by chronic urinary tract infection and inflammatory changes in the lower urinary tract. Previous transurethral resection of the stenotic bladder neck did not provide a lasting positive effect and resulted in recurrent bladder outlet obstruction 5 years later. The authors believe the recurrence of bladder neck stricture is due to impaired tissue regeneration in the resection area, coupled with recurrent infection and ongoing enzymuria. A relevant solution to this problem is a modified pancreas transplantation technique with enzyme diversion to the small intestine.
